If you're underwater for over 15 seconds, you'll see this on your HUD. It means that you're drowning and you're losing health fast. The only cure is to get to the surface asap. If you lose all your health you'll die. Once you get to the surface, your lost health will gradually replenish until it returns to what is was before. You need to return to the surface for a second or so before you can return underwater without immediatly starting to drown again. This damage does not affect your armor.
