Missing enemies?

Can't find the 78th in The Palace of Hate. Been through the level 3 times now.

Aye, E4M4 is the toughest to find them all IMO.

Did you take the secret teleport past the giant chopping blade in the dark hallway? If so, you missed a trigger that spawns a fiend. You have to be as close to the blade as possible without getting killed. Lots of other monsters spawn in this hallway too, sometimes I gotta run back and forth from both directions.

There's a fiend in the secret with the pentagram that you have to shoot the hidden button on the arch in the main area.

Shambler spawns behind you at the very end before you exit.

Knight in the secret under the staircase.

Sometimes a knight starts roaming around the main area and gets lost looking for the player, I have to backtrack and find him.

Oh my God, this is the second time this has happened. I beat episode 4 and it was as if I never beat the previous ones!!!
