What's your Quake UPC/Source Code?

I searched around the net trying to find some kind of list of all the different upc and "source code" numbers for quake and I had no luck. I thought it would be interesting to document the different versions, their codes, where they were sold, and the version of quake. Where better place to start such a project than here, no? Forgive me if this has been done already also, as I am unable to find such a thread or database.

Let me start off with my own copies, both shareware and retail versions.
