I honestly don't know about the time setting, probably the after math of future wars, genetic mutations such as zombies and other creatures with a strange porno twist to each weapon's ammo that screams rides suchandsuch's rocket or nailed by... , or accepts the shaft of.. or raped with a broomstick.. etc.. something like that.. I would say a few years after 2012 a bomb shelter that was harvesting sexual deviants during a nuclear holocaust was invaded by ravenous zombies and only 1 survived and escaped, and now must kill everything it sees.

What time is Quake set in?

I also think each dimension is in its own time-era:
-Spawn come from a really early primordial era since evolved as such
-The shamblers , scrags and Vores come from a completely timeless place that has never changed
-The Knights came from a neo-gothic place that since even lost its own way in psychotic service to the Quake Monster. The fiends probably came from the same time.
-The ogres are neanderthal-esque troglodytes that were given chainsaws and crude guns by someone else
-The base soldiers are simply converted earth soldiers from Quake's main timeline (as far as the Quake guy's concerned, they're his former comrades or ordinary soldiers turned bad)
-The zombies had been preserved for thousands of years and fermented back to life, or something...Check out my awesome Deviant art page!

The Story
QUAKE
Background: You get the phone call at 4 a.m. By 5:30 you're in the secret installation. The commander explains tersely, "It's about the Slipgate device. Once we perfect these, we'll be able to use them to transport people and cargo from one place to another instantly.
"An enemy codenamed Quake, is using his own slipgates to insert death squads inside our bases to kill, steal, and kidnap.
"The hell of it is we have no idea where he's from. Our top scientists think Quake's not from Earth, but another dimension. They say Quake's preparing to unleash his real army, whatever that is.
"You're our best man. This is Operation Counterstrike and you're in charge. Find Quake, and stop him ... or it ... You have full authority to requisition anything you need. If the eggheads are right, all our lives are expendable."
Prelude to Destruction: While scouting the neighborhood, you hear shots back at the base. Damn, that Quake bastard works fast! He heard about Operation Counterstrike, and hit first. Racing back, you see the place is overrun. You are almost certainly the only survivor. Operation Counterstrike is over. Except for you.
You know that the heart of the installation holds a slipgate. Since Quake's killers came through, it is still set to his dimension. You can use it to get loose in his hometown. In RMQ, we more or less think of the ogres as a brutal, tribal society, who nevertheless are able to make a variety of crude but effective weapons, and probably also crude armour and traps and other things.
If you come across some crude but brutal stuff in the dungeon, which might or might not work at the time, it was probably made by the ogres. They love making stuff to kill things - primitive crushers, nail shooters, and so forth. They do it just so they can laugh brutally when something gets squished, spiked, or otherwise mutilated.
There have been cases when their contraptions malfunctioned though, quite many cases actually, and the ogre ended up some other monster's breakfast.
Their society seems to consist of tribes or large kindreds, led by a tyrant, who may change often. Kids are plenty, but not many survive their early years. Women are usually the property of their men, or slavers, which might be one and the same, and even less of them survive.
Ogres are prone to spontaneous, deadly struggles among themselves, unless conditioned by others to work more or less as a team.
In the dungeons of RMQ, there have been sightings of grenade, nail, rocket and berserk ogres (it's always hard to tell which variety you're up against, until you are exploded, nailed, pasted across the wall, or become an ogre victim in some other way), and there are rumours about older, nastier ogres ... but no marine lived to tell about those.
Yeah, nasty buggers ... those ogres.
